Support Coordination
Voice of care support coordinators will help find and engage suitable service providers that help you to work towards and a achieve your goals. Our coordinator will help you to set up service agreements with your chosen service providers. It’s time consuming to do this task on your own, that’s why our support coordinator comes in to help.
Having our support coordinator will assist navigating through the NDIS process and help you feel prepared for any NDIS meeting you might have as they will collect any reports from the therapist or providers and complete their own progress report for the NDIA planner to review and make their funding determinations for your following plan.
